Women, Infants & Children

Florence County WIC office Hours are 8am-4pm on the First Wednesday of Each Month (exceptions can be made). Call 715-528-4837 or email Sara Jerue, Amber Kolberg or Ann Price for more information or to schedule an appointment. The Women, Infants and Children Program provides food and nutrition information to help keep pregnant and breastfeeding women, infants and children under 5 years of age healthy and strong. You may qualify if you live in Wisconsin, are pregnant, breastfeeding or a new mother, you have an infant or children under 5 years of age, you or your children have a health or nutrition need and have a gross household income that is less than or equal to WIC income guidelines. Farmers Market Nutrition Program

Farmers Market Nutrition Program (FMNP) is now available June-October.  If you are currently enrolled in WIC and interested in receiving $30 in FMNP Benefits, please contact Sara at the WIC office at 715-528-3426.

First Appointment

At the first appointment you will need the following: Proof of identification for person being enrolled, for example: o Birth certificate, hospital record, or crib card o Photo ID (school ID, county ID, driver’s license, passport, military ID) Proof of identification for parent, guardian, or proxy: o Same as above Proof of current entire household income, for example: o Current pay or unemployment check stub o Current Notice of Decision Letter (Food Share or Medicaid) o Forward Card o Child support check stub Proof of residence, for example: o Current bill o Rent receipt or lease with address included o Any document with your name and current address Proof of pregnancy (for currently pregnant women): o Signed statement from physician o Prescription for prenatal vitamins *The above are examples and other items may be acceptable. Please call the Health Department if you have any questions.
